Skilled Nursing Services
- Implements and coordinates plan of treatment initiated by the physician
- Assesses/evaluates patient’s status
- Consults/educates patient and family regarding the disease process
- Coordinates with other needed health care disciplines
- Administers medication and/or treatments as prescribed by the physician
- Applies preventative, therapeutic and rehabilitative nursing procedures
- Observes patient’s changes to the physician
- Evaluates effectivesness and outcome of care to achieve a succesful discharge/recovery
Rehabilitation Services
Physical Therapist
- Assesses therapeutic, rehabilitative, functional, and potential needs such as muscle strengthening, range of motion, mobility and gait training
- Sets goals and priorities for plan of treatment
- Directs therapy programs that provide preventative, rehabilitative and therapeutic procedures and techniques
- Instructs patient on home exercise programs to promote safety, good health and independent living
- Evaluates/educates patients on proper use of therapeutic equipment and prosthetic devices
Occupational Therapist
- Assesses patient’s occupational therapy needs
- Evaluates for muscle function, strength and endurance, visual coordination, written and verbal communication skills, self-care and ability to perform activities of daily living
- Establishes goals for communication and develops a therapeutic plan of care
- Instructs patient on activities of daily living
- Evaluates progress and reports to the physician
Speech Therapist
- Conducts diagnostics speech and language evaluation
- Identifies functional speech deficits and develops appropriate plan of care with the physician
- Provides direct speech therapy using specialized therapeutic technique and recommends communication devices, aids, and/or equipment
- Evaluates progress of therapy and reports to physician
Medical Social Services
- Assesses the patient’s psychlogical, social and resource needs
- Makes community referrals appropriate to pateint’s needs and serves as a liaison between families, patient and community agencies
- Assists the home health care team in recognizing significant social and emotional factors related to patient’s health conditions
Personal Care Services
- Provides personal care services ordered in the plan of care such as bathing, oral hygiene, skin care, prevention of skin breakdown, dressing and toileting
- Prepares nutritious meals and assists in feeding as necessary
- Takes vitals signs when ordered by the physician, and assists the patient in self-administration of medicine
